After conducting two study days (one year apart) in the early 2010s, the author was invited to offer a series of consultations to the trainees in Psychiatry in Kwai Chung Hospital, in Hong Kong. With ethic committee approval and care about confidentiality regarding patient anonymity, he has been recording and then transcribing these inter-professional consultations. These have continued for about ten years but he has been writing up the consultations of the years 2017, 2018 and 2019, a total of about 50. This account would be of interest to doctors training in psychiatry and any psychiatrist who is interested in gaining a deeper understanding of their patients; this will in turn help them formalise a treatment programme which will be more focused to the patient needs, more beneficial for the patients and giving greater satisfaction to the doctors - who often burn-out under the pressure and demands regarding their highly disturbed patients (pressure from patients, colleagues, seniors and managers). Although the book is informed by psychoanalytic and group-analytic concepts it deliberately excludes jargon; it is written in plain language so that it is accessible to non-analytically trained professionals. As each consultation is virtually transcribed, the reader can see how the understanding emerges through the consultation and it is not presented as a ready conclusion. Where appropriate, relevant bibliography is cited. The consultations deal with patients who present with acute but also with chronic conditions, seen as emergencies but also as long standing cases, in-patients and out-patients, suffering from a wide range of conditions (including depressive illness, suicidal, schizophrenic, alcoholic, traumatised etc.) These are not psychotherapy consultations although psychotherapy is considered as an indicated form of treatment for some (and contra-indicated in others).
